UnitedHealthcare Extends National Public Health Emergency

UnitedHealthcare Extends National Public Health Emergency Temporary Program Changes, they recognize that the persistence of COVID-19 cases across the country continues to have an impact on the healthcare system – both for COVID-19-related care as well as for regular preventive visits. To provide ongoing support to members and providers, UnitedHealthcare is extending many of the COVID-19 temporary programs, process, and coverage change through Oct. 22, 2020. Full details, including applicable benefit plans and service information, can be found online.

Implementation of these temporary changes may vary by the health plan and for self-funded customers. State-specific and Medicaid rules, regulations, date limitations, or exclusions may also apply.

UnitedHealthcare National Public Health Emergency Changes:

  • Cost-share waivers for COVID-19 testing-related visits, testing (diagnostic and antibody), and treatment.
  • Telehealth coverage for COVID-19-related services for in- and out-of-network providers.
  • Telehealth coverage for non-COVID-19-related services for in- and out-of-network providers.
  • Implementation by health plans may vary.

First Coast Service Options has instructed providers to bill professional claims for all telehealth services with POS on and after March 1, 2020, and for the duration of the PHE to Medicare with:

  • Place of service (POS) equal to what it would have been had the service been furnished in-person (example: POS 11 for office).
  • Modifier 95 applied to claim lines for services furnished via telehealth.

If you filed a claim with POS 02 you were paid the facility rate, which is a lower payment amount.

To correct the payment differential, providers can request an appeal of the paid claim to revise the POS and add modifier 95.  This option is available for only telehealth services furnished on or after March 1, 2020, and through the duration of the PHE.

QPP Participation Status Tool Now Includes First Snapshot of 2020 Qualifying APM Participant and MIPS APMs Data.

Quality Payment Program

On July 20, the Centers for Medicare & Medicaid Services (CMS) updated its Quality Payment Program Participation Status Tool based on the first snapshot of data from the Alternative Payment Model (APM) entities. The first snapshot includes data from Medicare Part B claims with dates of service between January 1, 2020, and March 31, 2020. The tool includes 2020 Qualifying APM Participant (QP) and Merit-based Incentive Payment System (MIPS) APM participation status.

What Does QP Status Mean?

If you qualify as a QP, this means you are:

  • Eligible for the 5% APM incentive bonus; and
  • Exempt from participating in MIPS.

How Do I Check My QP or APM Participation Status?

To view your QP or APM participation status at the individual level:

To check your 2020 eligibility at the APM entity level:

  • Log in to the CMS Quality Payment Program website. Learn how by downloading the QPP Access User Guide.
  • Browse to the Taxpayer Identification Number(s) affiliated with your entity.
  • Access the details screen to view the eligibility status of every clinician based on their NPI.

The Centers for Medicare & Medicaid Services (CMS) provided guidance in the April 7, 2020, Dear Clinician letter for using diagnosis codes Z03.818, Z20.828, and Z11.59 when reporting encounters related to possible COVID-19 exposure. First Coast Service Options, Inc. (First Coast) was erroneously denying claims as preventive/screening services when submitted with diagnosis codes Z03.818 and Z11.59.

Resolution

Effective for claims processed on or after July 17, 2020, for dates of service on or after January 27, 2020, First Coast will no longer deny claims when submitted with diagnosis codes Z03.818 and Z11.59, as long as all other coverage requirements are met for claims processing.

Status/date resolved

Beginning July 17, 2020, for dates of service on or after January 27, 2020, claims will no longer be denied when billed with diagnosis codes Z03.818 and Z11.59.

Provider action

Also, no provider action is needed. Because First Coast will adjust impacted claims.

 2% Payment Adjustment Suspended

According to section 3709 of the Coronavirus Aid, Relief, and Economic Security (CARES) Act temporarily suspends the 2% payment adjustment currently applied to all Medicare Fee-For-Service (FFS) claims due to sequestration.

The Coronavirus Aid, Relief, and Economic Security Act (CARES Act) provided additional funding to CMS for necessary survey and certification work related to COVID-19, of which $80 million in new resources will be available for states to increase surveys and get more COVID-19 Data.

To ensure effective oversight is achieved, CMS will allocate the CARES Act funding based on performance-based metrics. States that have not completed 100 percent of focused infection control surveys of their nursing home by July 31, 2020, will be required to submit a corrective action plan to their CMS location outlining the strategy for completion of these surveys within 30 days.

If, after the 30-day period, states have still not performed surveys in 100 percent of nursing homes, their CARES Act fiscal year 2021 allocation may be reduced by 10 percent.  Subsequent 30-day extensions could result in an additional 5 percent reduction. These funds would then be redistributed to those states that completed 100 percent of their focused infection control surveys by July 31.

Utilizing the CARES Act funding, states will be required to perform on-site surveys of nursing homes with previous COVID-19 outbreaks and will be required to perform on-site surveys (within three to five days of identification) of any nursing home with new COVID-19 suspected and confirmed cases.

To help nursing homes implement infection control best practices, CMS will provide technical assistance through Quality Improvement Organizations (QIOs). CMS and the Centers for Disease Control and Prevention (CDC) will continue to monitor the data it receives through the new nursing home COVID-19 surveillance system to identify nursing homes with outbreaks and work with Governor’s offices and states to keep nursing home residents safe.

The Department of the Treasury and the SBA released the Paycheck Protection Program Loan Forgiveness Application with detailed instructions for the application.

To apply for forgiveness of your Paycheck Protection Program (PPP) loan, you (the Borrower) must complete this application, and submit it to your Lender (or the Lender that is servicing your loan).  Borrowers may also complete this application electronically through their Lender.

The application has the following components:

  1. the PPP Loan Forgiveness Calculation Form,
  2. PPP Schedule A,
  3. the PPP Schedule A Worksheet, and
  4. the (optional) PPP Borrower Demographic Information Form.

All Borrowers must submit (1) and (2) to their Lender.

The Borrower must retain all such documentation in its files for six years after the date the loan is forgiven or repaid in full, and permit authorized representatives of SBA, including representatives of its Office of Inspector General, to access such files upon request.

  • On May 19, 2020, the Centers for Medicare and Medicaid (CMS) published the payment rates for the new Covid-19 antibody testing codes.  Please see below.
  • On March 13, 2020, the American Medical Association (AMA) created CPT code 87635 for infectious agent detection by nucleic acid tests.
  • On April 10, 2020, the AMA created CPT codes 86769 and 86328 for serology tests.  Laboratories and qualified physician practices performing these tests may bill Medicare for services that occurred after their respective effective dates.
  • Local Medicare Administrative Contractors (MACs) are responsible for developing the payment amount for claims they receive for these newly created HCPCS codes in their respective jurisdictions until Medicare establishes national payment rates. The payment amounts are identified below.  As with other laboratory tests, there is generally no beneficiary cost-sharing under Original Medicare.
